Likely this was an intentionally humorous restroom sign. Personally, I would get a chuckle out of seeing it, and a lot of popular clubs/bars I’ve been to have unique or funny restroom signs.
The colors are too solid, there isnt enough variation or reflection. And the white “rounded rectangle” has a clear stairstep or jaggy effect on the corners, as well the default rounding present in the Photoshop CS3 rounded rectangle tool.
It’s a pretty obvious Photoshop, and I only have a passing familiarity with the program.

Maybe this is something similar warning women to watch out for guys leering at them over the top of the partition and the “stylish” thing is just poorly juxtaposed? That is because women in Japan (girls really) are regularily molested in subways in Japan because nobody will do anything about it. I seriously doubt there is a large number of guys really interested in a girl on the toilet that would warrent this (clearly shooped) sign.

I’m thinking that the guy was originally in a wheel chair, and someone removed the chair part and repositioned him looking over the edge. I have no opinion as to whether this was done in Photoshop, or someone was able to re-arrange decals, creating the sign in “real life.”
Maybe the little wheelchair dude hit a kerb right before he got onto his picture? Those things can travel some, so its not beyond the realms of probability that one could launch oneself pretty goddamn far like that….
